Wher to get a DOT physical long form.

    You know what's really funny? Our grade school teachers did that nose on the chalkboard thing when students talked too much in class. Don't ask me how I know how that really works. :biggrin_2554:

    A long form is supplied by the office in which you take your DOT physical, a normal person can not get a hold said document legally. It must be taken at an office that does DOT physicals or it will not be valid, average price is $70 which is good for 2 and must be taken every 2 years. And to the second part the driver copy comes with the long form you must carry it with you at all times in a commercial vehicle, I carry it at all times anyway.

    As has previously been mentioned, a blank long form can be purchased at most truck stops or downloaded off the internet for free.

    I've never paid more than $60 for a DOT physical.

    NEVER go to your own personal/family doctor to have your DOT physical done. You don't ever want to find yourself medically disqualified over a question you may have had or a discussion which may have occurred between you and your regular doctor. Go somewhere that does routinely does DOT physicals, and your DOT physicals should be the ONLY times this doctor or clinic EVER sees you. Answer any questions they have honestly, but do NOT volunteer extra information. The DOT physical is NOT the place to discuss your athletes foot problems or any other health issue you may be having. You should try to come across as the picture of perfect health during your DOT physical...your livelihood depends on it.
