So I'm coming up on the end of my first year of driving in Mid January, and the end of the calendar year is near as well.
I hear I need to fill out a form for my employer listing all, um what, tickets and log book violations.
I hear about this DAC report. Does one use the DAC report to fill out the yearly form?
And I am told by

that I can get my free copy of my report from these jokers:

But it takes 15 days to get the report! And my employer needs it by Jan 1st (I'm told). And I would have to catch up with my mail somehow.
Is there an easier / quicker way?
Can someone provide a quickie guide to this thing? I tried searching for threads on this here to no avail.

It is not up to you to supply a DAC report, only a DMV printout, and 10 year employment history. If they want a DAC they can go get it.

Are you telling us you can't remember any tickets you've had in a year?
The company supplies the form, you simply fill it out.

Yup....by memory. Of course, if you had multiple violations, you may need a reference for accurate dates. My last (and only) ticket ever in a CMV just happened to be presented to me on.....April 1st.

It's not that I can't remember, it's that California can't!
I got a citation in CA my second week out. Pure idiocy on my part. I didn't quite grovel and beg, but I was ready to. I got a yellow piece of paper out of the deal. I call the number as soon as it says to do so. They did not recognize the number, please call back in a month. A month later, the machine says that some citations are unavailable due to furloughs, call back in a month. Again, same thing. Next month, the system does not recognize the number. Same answer two more times. My state has no inkling of it AFAIK.
Either the nice Police Officer lost it or the state did.
So I'm thinking my company doesn't want to hear about it. -
If your were sited for a misdomeanor violation they have up to one year to set you a court date.you shoould check your copy and see what box is marked ..

Have your 14 year old nephew make one up. I can't believe a company is expecting you to supply with the DAC report and they give you a deadline for it. Do they also want you to pay them for orientation and training?

Drivers annual review, required by the feds, on your hire date anniversary. You have to list all accidents or citations. If your citation was dropped or you won in court, you do not list.
As to the DAC, you do not use it to fill out the DAR. The info on it is added by your company, so you wouldn't need it to list on the form.Last edited: Dec 16, 2012

Calm down hotshot and reread his post. The company dosen't want him to supply his DAC. He didn't understand what a DAC was and questioned if that was the form they were wanting. Look at Buzzarddrivers post. Perfect explanation.spacester Thanks this. -
it's an annual tradition to have to fill out that form on tickets received in the past year. it's an insurance issue. not to do with dac what so ever.
why we have to fill out that form is beyond me. since the company will be pulling up your driving record anyways.
i don't know anything about some regulation if that may be the case. i just keep getting told the insurance company needs it.spacester Thanks this.
