two things could of caused this that I can think of, not enough air for the fuel you was delivering or air intake temp was to hot. may have to install a oversize CAC. or run fan before it automatically kicks on.

Have seen that with 4 engines last year and one was stock. These failed under 1000 kms after overhaul. I would replace the liner pack and run it. The two I sent in for warranty and have not had an issue since. Make sure you have the stock injector trim files in.

How do you check this, that's ecm right? Would those typically be changed if the ecm was reflashed to a different file, or is that a separate thing?

Not famailar with that engine at all but, after looking at your pics it reminded me of an engine that had just went out the door with a major rebuild and came back in in just a short time with that same problem. The man that put it together "6 cyl international" had used excess lube on the lower cylinder O-rings when installing 2 of them, the excess lube expanded and squeezed the lower part of the cylinder reducing oil clearance, and of course the piston started transferring to the cylinder. Not saying that is your problem, but you did say it was on one cylinder?

that right there explains a lot. with a pdi tune and 6999 trims. take the 4 digit # off of each injector and load in to the ecm accordingly. cat did have or still having an issue with piston to liner clearances on some of the liner packs they have been selling.
