It has been said before but I'm going to reiterate.
CARRY YOUR VALID MEDICAL CARD!
DO NOT RELY ON YOUR PHYSICAL DOCTOR TO REPORT/RECORD YOUR MEDICAL CARD FORM WITH DMV/DOT. MAKE SURE DMV HAS YOUR UPDATED CARD.
IF YOU DON'T HAVE TIME TO GO TO DMV, CALL THEM OR GET AN ONLINE COPY OF YOUR DRIVING RECORD. -
My short form is in my wallet. Long form is in the truck's permit book. Not all DOT know everything. Sometimes, they are just as lost when we are when it comes to the ever changing regulations the FMCSA puts out. While Oregon will say you don't need something, Washington will require it, and Idaho has no idea. The old saying "it's better to err on the side of caution" always applies to everything.

The actual med cert now expires in 15 days...time enough to allow the state to enter the copy you sent in.
BUT NJ Motor Truck ASSN recommends that you keep it just in case... -
Does the person doing your DOT physical always have to write up a "long form"?
They gave me a short form I guess or a plastic card. Is the long form extra? -
The company will want to see the long form, they need it for your driver file along with a copy of your card.

If so, I can just go ask for a copy? -
