I would think its the throw out or also called the release bearing. It may need some grease. When you take the free play out of the pedal you are making the fingers that move the release bearing touch it is all, you are not actually moving it towards the clutch brake until where the free play ends. Guessing the release bearing is the issue in one respect or another.

The thing with the clutch release bearing is that it should be greased often but take care not to over grease it. The release bearing in these clutches are not like a car where they only rotate with the pedal pressed. HD clutch release bearings turn all the time but are only under load when the pedal is pressed.

I have went over everything. Been serviced great, just seems like everything has fell apart at once. :(

I just found out when I got it hooked up to ET that I am pushing almost 400 HP and 900 some ft lbs of torque. That part did make me happy though. lol

We have only bought the truck a month or so ago and went through and greased everything. I was thinking maybe the lack of grease before on the clutch release bearing and then it got grease its put it out. I have heard if they haven't been greased in a long time, don't grease them. Will put them out.

900 ft lbs of torque? Should be a lot more than that. 400 horse should get you at least 1450. The charge air boot sounds like a pretty good thing to check, hearing it through the shifter boot sounds like a very likely suspect. Maybe take the shifter boot out and drive it and see if you can pinpoint the noise better?
