Carrier Package

Did you ask the company what they need/are looking for?

 

Are you submitting to a broker? If so, they should have guidelines for you. In general, what we include are:

- Cover sheet of our Company Credentials, which states

1. 1. our corporation name and year
   2. mailing and physical address
   3. phone and fax numbers
   4. email addresses
   5. EIN#, MC#, and DOT number
   6. Bank information
   7. Fleet size
   8. References

- MC Authority letter
- Certificate(s) of Insurance
- Completed W9

Those are automatically sent to any broker with whom we're setting up a contract, in addition to their carrier packet forms.

I personally wouldn't give out my bank information.

 

Just general info. What bank, adress and the phone number. No accounts.

 

I have it as a pdf bundle, shoot it from my laptop or smart phone as an email.

 

Exactly -- just name, city and state of our bank. Sorry, I probably should be been more specific there!
