Refuses to give Long form because it's "their property". Assumption~because they paid for PHYSICAL? IDK
If employer will not give you DOT long form go to doctor that did physical. It's your health information.
According to FMCSA:

• The medical examiner is required to provide the driver with a paper copy of the original Medical Examiner’s Certificate, form MCSA-5876.

Your Medical Records

The Privacy Rule gives you, with few exceptions, the right to inspect, review, and receive a copy of your medical records and billing records that are held by health plans and health care providers covered by the Privacy Rule.

 

Doctor sent it to empower.

 

You still have the right to go to that Doctor's office and request/receive a copy of the form.
