Doctor sent it to empower.
-
You still have the right to go to that Doctor's office and request/receive a copy of the form.
-
Don't you carry a copy with you at all times?
-
Dot only requires short firm whenbon road or for scalehouse
-
Wow cant type... short form, and when on road
-
I find it weird that the doctor would send it to the employer especially since we all have to self certify now. How are you supposed to self certify if the employer has your long form?Maybe we do it different here in California.
-
No they do not.. Once you receive your Medical form you need to submit it to your Local DMV where you hold your CDL license and Self-Certify. This has been in place for several years.
