    When you bypassed the supply, did you hook up to the fuel ports on the tank, or did you just put the hose in the filler hole? Possibly a cracked stand-pipe? Could have bad fuel? water, algae, gelling issues in the cold... Possibly a cylinder head issue but that would be my last choice, it seems like you've covered most of your bases, no codes? only happens when cold? Post back with results and observations please

    Hey did you ever find your problem my truck is doing the exact same thing and can’t figure it out replaced egr valve and egr cooler and nothing fixed it so can you tell me what was the fix

    So my problem was fuel issue. This truck had separator on fuel filter housing and at some point someone decided to put cigars in the tanks which blocked ecm cooler so I took off all fuel hoses and started blowing them out till I removed restriction and it was good

    if you include the parts that were changed at the customers request because he was trying to rush me to get truck running as soon as possible, ( when he finally gave up pushing was the day i figured it out)

    So injectors 4000
    fuel filter housing 500
    drain all fuel dispose send out tanks for cleaning and reassemble. 1400
    clean all fuel lines blow out everything and reassemble. 450
    my headaches of having to mess with this for 2 weeks till i figured it out even when volvo couldn't. Priceless.
