Truck in Ottawa, Ontario lost a wheel, which struck another vehicle and unfortunately killed its driver. Gotta check those lug nuts on the pre-trip.

Check wheel seals, and look for metal fines in hub oil as well. Bad deal. Hate to see this stuff happen.
lovesthedrive and misterG Thank this. -
I think Ontario will fine the truck's owner a large sum of money, plus impound the truck for 30 days.

#####. JUST went over this with the wife yesterday before we hauled out. Very important part of PTI. Sad. sorry.~

The article is not very clear. It says "dual tire". Are they refering to the outside dual coming off or the pair because of a hub nut/bearing failure?
-
Hmmm...interesting, and not sure. Didn't appear to be "super single" issues. @cnsper? didn't you go thru something like this recently?
-
Yes only it was the lug nuts. They can be tight by hand but still be loose by tire standard.
Dye Guardian and G13Tomcat Thank this. -
Being hit by a loose tire is my #1 fear on the road. I know exactly how many incompetent boobs there are out there wrenching and doing tire work these days.

I was getting new tires on my trailer. They showed me that the inside rim was ready to come off on 2 sets. The hole had rusted and the nut or bolt with the flange could almost pass through the hole. These are piloted hubs

If the reporting is correct and it was a dual wheel, I am inclined to believe it was bearing failure and an axle shearing off, leaving the dual set intact.
