I agree with you that 2 spread axles are not a axle group. By that definition that would make most 53' spread axles with a rear slider illegal in almost every state east of the Mississippi.
I have found that to not be the definition most states go by at least for kingpin the rear axle group purposes.
I would typically only close my spread in Ny, Fl, Ct, Ca, and some of the states that are real tiny on the map but I have never been to those. -
You just don't take more than 34k thru those states?

I find the state rules to be intentionally confusing as a sly border tax.
Are there any states that wont allow a 48ft fixed spread axle step deck? Im under the impression that with a 10ft spread 20k per axle is permissible. Are there any states that dont honor this? -
Well......there are plenty of roads in PA that won’t allow 48 ft trailers. And still more that won’t allow 102”. But the answer you’re looking for I think is 48 ft can go anywhere which is more or less true and it can axle at least 40k in the lower 48 but not in parts of Canada. Clear as mud?

Im guessing this is why i see a lot of tail overhang on the back of 53' fixed spread trailers.. To keep the kingpin span at 41 ? -
Okay, say for example, you are taking a load from Houston to Jacksonville. You're 11.8 on the steers, 33000 on the drives and 35000 on the trailer. Before you hit the Fla line, you stop and close the spread. When you do that, it will move 2500 lbs from the drives to the trailer.
Your new weight will look like 11.8, 30.5 and 37.5, right? Florida requires you to close the spread, but they will give you 44k on the tandem. The easiest thing that you can do is close the spread before you load and try to load your drives and trailer evenly. But if you can't, run with the spread open until you get to Florida. If you are legal in Texas with the spread open, you will be legal in Florida with the spread closed.

CT is one. I don’t run west so I forget but I think CA might be 40.
