You can borrow my sharpy, jot it down on a 8.5 x 11 envelope from dollar general,

 

Better to create your own that has only the info you need in a flow that works for you.

20 different guys build 20 different "trip envelopes" they will all be different and not even close.

 

Why not just edit that one?

adobe acrobat pro not reader. i'm going to convert it now, but it's really a crappy scan.

 

i can convert it, but this board won't allow an XML or xlsx file extension upload.

 

Acrobat pro will.let you convert, edit, and even sign docs. Costs about $30/yr IIRC. Well worth the money.
I use it on all.my rate confirmation sheets and other docs. Nearly paperless now in the office.
