    Better to create your own that has only the info you need in a flow that works for you.

    20 different guys build 20 different "trip envelopes" they will all be different and not even close.

    I got it as a hard copy and converted it to pdf(by scanning)...

    i can convert it, but this board won't allow an XML or xlsx file extension upload.

    Acrobat pro will.let you convert, edit, and even sign docs. Costs about $30/yr IIRC. Well worth the money.
    I use it on all.my rate confirmation sheets and other docs. Nearly paperless now in the office.
