I see Western Express is one, do you know of any others?

 

DAC report carries some weight, but MVR carries more.
You're MVR is clean, so that helps.
Don't go by the websites; some carriers look at situations like yours on a case by case basis.

I have currently been away from trucking for 1 year, I had 5 preventables in 10 months. I am curious if anyone will take a chance on me ?

 

Are there any companies in the Memphis area that anyone knows of that give second chances?

 

How long ago were the preventables and were any serious wrecks?

 

The last preventable was in February of 2019, I dropped a trailer at a truck stop. No serious accidents, mainly slow maneuvering problems

 

So, your MVR is clean then?

If your MVR is clean; just list what's on your DAC report as incidents, not accidents.
Watch the dates on job applications; Some only ask for incidents/accidents for last 3 years or last 5 yrs. etc.
