Address on Physical

    I'm changing my address this week and was curious if i need to change the address on my medical card. Safety dept or the clinic couldn't seem to answer this question.

    Nah, there is nothing I know of. You self certify and change your drivers license address which puts them together on the DMV computer with your new address. That's all they need.

    Where I live we just self certify...I literally snap a photo of my New Card with my iPad and send it to the state....I imagine I would just re take the photo an send along a new address the state will update it.

    I just dont want DOT saying something because my med card address isn't my current address

    how is dot going to know?

    they ask, is this your current address. you say "YES SIR".

    it's not liike they ask for a copy of your utility bills. and they're certainly not going to drive across the country to go knocking on your door to see if you answer.

    My med card has our address on it. They will know when it doesnt Match my license

    The only regulation regarding updating a medical card is if your health changes then you must recertify. It is only a certificate of health. It is not a form of ID. Again it's attached to your drivers license in the system. Next year you won't even have to carry your medical card.

    Forms of ID such as your CDL are state issued and require a correct address to verify you live in that state. A medical card is a federal document and can be obtained in any state. Safety and the clinic doesn't know the answer because there is none. You are worrying over nothing.

    You can do it in person, online or mail this form. You have 10 days from the move. http://www.flhsmv.gov/forms/71117.pdf

    Change of address:
    CDL, registration, title, voters registration card, employer, IRS, banks, insurances and utilities. You can get a change of address kit at the post office or go to the individual websites.
    Basically anyone that sends you mail.

    Florida, being such a wonderful state, will only allow address changes in person.

    i think utah is one also. you have to provide proof of some sort.

    i think the rules changed when obama first took office and the gooberment started cracking down on immigration. for awhile. EVERYONE had to provide a legal ss card or birth certificate. along with a utility bill. even if it was to just renew your license. we all had to provide proof of citizenship and residency.

    not quite sure on the procedure now, as i haven't changed my address.
