Hey have a 2016 dd13 with code SPN: 5361 - Engine Cylinder #4 Fuel Injection Quantity FMI: 18 - Low—moderate severity. Unit had a new fuel pump and all new injectors installed. Changed injector number 4 again but code still comes back after driving. Injector harness changed. Compression in all cylinders is good. Any ideas?

    What's the fuel pressure?

Were the injectors replaced because of this code and problem or did it start after the injectors were replaced? Really need to have a scan tool or laptop connected and see what the injector pulse is for each. You could swap #4 injector with another one and see if the problem moves to the other cylinder. Also swap the rail to injector line (#5 under Probable Causes is the line) if they are the same. I would also look closely at the rocker surfaces.

Rail pressure and desired rail pressure is fine, there are no codes for rail pressure

The injectors were not replaced bcs of this code, the injectors were replaced bcs of a no start condition. The valve adjustment has been done twice and injector 4 has been changed again with a new high pressure line

I would check for an intake air leak on #4. "Motor control module (MCM) adjusts injector pulse for smooth idle – max value reached." This implies a rough idle. What type of compression test was done, relative or using a gauge? Again, realIy need the data by using software (scanner or laptop) to see what is happening with #4. I read the first time that #4 injector was changed after the code occurred. But swapping #4 with another cylinder does this (however unlikely to be your problem at this point) the other 5 injectors are not setting any codes, By taking one of these and placing in #4 1 of 2 things happen 1 - code moves to other cylinder and then you know injector IS the problem 2 - code does not move but you know for sure injector is NOT the issue.
"Rail pressure and desired rail pressure is fine, there are no codes for rail pressure" How do you know the pressures are fine? Just because a code does not set does not mean there is not a problem in that area.

A relative compression test was done, I replaced the injector bcs I had another unit with the same code and the injector replacement fixed the issue. The desired pressure and actual pressures are equal which tells me it’s very safe to assume that the pressures are fine if the ecu is satisfied with it and if pressure was an issue there should be multiple cylinders having an issue not number four continuously. As far as intake leak don’t see how that would be since it’s a one piece intake manifold and the valves on that cylinder adjusted fine. Think it could be the ecu not energizing that injector correctly ?

Found this on a technical forum. Similar code to yours.
    2016 FREIGHTLINER CASCADIA SPN 5362 FMI 16…
This code is defined as engine smoothness control cylinder number five quantity deficit. This could be a faulty injector, faulty MCM, faulty injector harness, programming and software related problems within the MCM or electronic control module for the engine by definition, or possibly internal mechanical problems within your engine that would cause compression related issues and poor combustion. I have the Detroit diagnostic software connected to this engine to run through all your injector tests.

NOTE: This fault is caused by low power on cylinder #4. Motor control module (MCM) adjusts injector pulse for smooth idle – max value reached.
This suggests a mechanical problem and the injector adjusts to compensate. The top 2 probable causes are cylinder or valve train. I was referring to a possible gasket leak between the intake manifold and head. Easy and quick to check to rule that out.
