The consequences of willful negligence are not accidents.

It takes all kinds 1135. There's more to the story pertaining to his personal life. A downward spiral. I actually feel sorry for the driver. Life has many turns along the way.

I wanted to say something controversial. I hear some people say something like "If I stay good and follow the rules, trouble will never happen". It is not true. Dealing with big machinery, accidents could happen even if you do try to follow every rule. This is the reason to be even more vigilant and paranoid, looking out for every trouble and taking time to assess risks in anything you do.

Thanks for sharing okie my condolences go out to all that were involved. Especially you and the deceised. Don't blame yourself. I share the same values and have experienced a similar situation.
When people make mistakes out here in this industry people die.
Thank you for sharing your thoughts I would have felt the same.
Your a human, and a good one at that. Again don't blame your self.
Guys if you ever come across such a situation as this, were your actions can save a life, go do it. You'll probably just do that, save a life that is. That way you don't haft to live the rest of your life thinking about what you could of done or what you'll can do next time.
Take care okei, again my condolances go out to you, your not alone.

Many years ago, when I was a young man in the U.S. Navy, there was a Hull Technician, (pipefitter & welder), who would actually weld metal without his safety goggles on. He was trying to prove to the guys that he was "MAN" enough to do this. Eventually, he was disciplined by the commanding officer for this type of behavior.

Saw a man get killed when some guys on a project decided to try tow a big truck with chain that was not rated for towing heavy equipment. The chain exploded and tiny pieces flew like shrapnel. One piece entered a guy's throat and lodged in his chest. He died a few hours later while being treated.
