That's what I thought.

 

When I was sent home from TMC I had some questions about my physical. I wanted to know if I had failed or if they had submitted the results at all. This was important because I had already completed a physical in MD and passed, so if they didn't submit the results I technically had a passing physical that was valid.

As it turns out, they pended me for 45 days to get the sleep test. But I had to talk to 4 different people at the clinic that does the physicals because they kept trying to tell me to go to the company for results. It took 4 tries to get someone who understood HIPAA. Just because the company paid and I signed the form to allow them to see the results, doesn't mean they own my medical records.

I finally put it to them like this: If I brought a homeless man in off the streets for a physical, and paid for it myself, who owned the medical records?

VQBIC i worked for tyson and They don't give it to you cuz They Paid for it. You have no need for it...

 

He should get a copy, regardless of what some company monkey thinks.

 

Why? He has no use for it as a company Driver

 

If he requests a copy it is illegal for them to NOT give it to him. It is his medical record. Who paid for it doesn't matter. Company policy does not over rule federal law. It's simple really.

I meant after self certify, u are required to keep short form with you in truck

 

I "believe" you need the whole thing on your person. Mine is 5 pages, which I reduced to 1 page.

What's the short form?
