So if thats the case why do i get 2 forms? 1 being wallet sized?

According to what i just read...In the interim, FMCSA has advised both its staff and roadside enforcement personnel to accept proof of a driver’s medical certification status and expiration date on a CDLIS MVR,
Key word here IS ADVISED, they are not required to take the mvr as proof, so ill just keep carring mine. -

If you do not Self-Certify with your State and get pulled over you can be cited if you only have a paper copy of your exam. What is on your States record is what they will go by, not what is in your hand. I suggest you verify your information with your state. For example a driver posted on here not too long ago that what was on the States records was key punched in incorrectly. Even though he had his paper copy he got placed OOS and a ticket for this. Until the State got his information correct he was not able to drive.

When did this change occur?
This change became effective on January 30, 2012, with full compliance due by January 30, 2014.
On or before January 30, 2014, all CDL holders must declare to their State Driver Licensing Agency (SDLA) that they only operate or expect to operate commercially in 1 of 4 possible categories with their CDL.
Effective January 30, 2015 FMCSA has verified that every State is posting the basic, minimal medical certification information which allows a motor carrier to validate if a driver is medically qualified. The basic information consists of the driver’s medical status and the expiration date of the medical examiner’s certificate.Lonesome Thanks this. -
I just renewed my med card in December. I did not receive a wallet size card. I received my long forms which I took to the DMV to self certify. They updated my records and gave me a receipt with a new expiration date. My long forms are filed at home and I carry the DMV receipt with me.

Then why is it that Maryland's finest consistently ask for mine at inspections?

I can not remember the last time that I have been asked for long form. I know that when I was with Swift some time back they gave me the long form to take to my DMV and when I started where I am now, they took the long form a month ago and they sent to my DMV
