How many of you carry with you a sheet of emergency contact information (employer name/phone number), insurance information, and information about who to contact in the event of a collision resulting in your inability to speak for yourself and medical decisions need to be made?

I have such a list in my wallet. it has next of kin, medical info.
I also keep a copy in my briefcase.
My family has been put wise on how to confirm anyone calling, as in the case for instance, should my wallet (or briefcase) ever be stolen and someone tries to scam them. -
Hopefully they can get to my phone and call MOM or DAD, the Ottebox should hold up in a crash.

Might be a good time to also remind everyone to keep your emergency contact info updated with your employer, as well. Lot's of times phones and wallets are the first items to be lifted when a scene turns ugly and the only thing LE has to work with is carrier name and truck number.

Google (Android) has a way you can add certain numbers to ICE Group (In case of Emergency)
I'd imagine Apple/Iphone has something like that as well. -
In my phones contact list I have my wife name preceded by the letters ICE (In Case of Emergency). First responders will check for this when looking for contact info.

My contact sheet is based on one used by many members of the Gold Wing Road Riders Association.
ICE contacts in phones are great if you don't lock them preventing accidental "butt dialing".
