Are you people saying that when you are picked for a random drug test you need to do it on the spot, right now? I'v never had this. The only times, I was selected for random, it was done when I came back from a trip, a few days after the selection. Always, at the clinic near home.

You have to go and take the DOT drug screen within a certain amount of time once notified by the employer yes.
Failure to do so in that time frame qualifies as a failure/refusal -
For one company I was always notified as soon as I completed a load. Supposedly to send me to a local collection place with adequate place to park truck/trailer. Sometimes there was. Might be a suicide left turn lane in front of the place.
Worst was a place next to the Angels stadium in Anaheim. They thought truck and trailer meant something like their landscapers used. I just parked in the street with my flashers on and went in to do the tests. People inside told me they would tow truck if I left it there but I didn't care. If the company wanted to send me to places like that then they could pay for a tow.
