when you finished your physical the doctor will fill out a small card that you can put in your wallet or somewhere of easy access its the same as the long form just wallet size

I'm confused as to whether I needed my physical or not. I can't find any documentation that states it's needed for the permit, but when I went to the DMV on Monday, they said I did.
I went ahead and did it, my Dr has been bugging me about getting one anyway. With my insurance it was only $20 and I got a figure stuck up my butt so it was worth it
I'm going to take the permit test tomorrow so I'll find out for sure when I get there.

When I took my exam, I asked for a CDL booklet at the DMV office while I stood in line (I walked in without an appointment). I think I was in line for about 20 min. For me, that 20 min of study was a huge benefit as several questions came from the information I had just read through. Good Luck. -

I went ahead and had a full physical done while I was at it. Did the 'Turn your head and cough thing' too. My current insurance benefits run out at the end of the month so I thought I would use them upgarciamag Thanks this. -
