    when you finished your physical the doctor will fill out a small card that you can put in your wallet or somewhere of easy access its the same as the long form just wallet size

    Are you sure about the DOT physical being needed in FL before getting your permit? I ask because I got my permit in FL in early January and no DOT physical was requested. Regardless, as mentioned already, PRIME will do their own. Also my experience is in line with WVA. Make sure you have the correct paperwork. Original BC, your CDL permi- if you have it, Passport- if you have it, and DL. Everything you did online (application - work history, references, Driving Record..ect) will be redone on the first day :biggrin_2551: so have that information handy.     Hi Soponno,

    I'm confused as to whether I needed my physical or not. I can't find any documentation that states it's needed for the permit, but when I went to the DMV on Monday, they said I did.

    I went ahead and did it, my Dr has been bugging me about getting one anyway. With my insurance it was only $20 and I got a figure stuck up my butt so it was worth it :biggrin_2556:

    I'm going to take the permit test tomorrow so I'll find out for sure when I get there.

    Ha, no butt check at the PRIME DOT physical so maybe you got that little extra bit as a gift.

    When I took my exam, I asked for a CDL booklet at the DMV office while I stood in line (I walked in without an appointment). I think I was in line for about 20 min. For me, that 20 min of study was a huge benefit as several questions came from the information I had just read through. Good Luck.

    I went ahead and had a full physical done while I was at it. Did the 'Turn your head and cough thing' too. My current insurance benefits run out at the end of the month so I thought I would use them up
