    Mechanic got back to me today. Fixed the problem. He swapped out the injectors from 5 and 6 and put them in 1 & 2. He says truck has been running great for an hour and a half.

    Should I be balancing injectors everytime I change them out?

    Sorry, I was away yesterday. If all he did was swap injectors around, I would say one wasn't seated properly and combustion gas was getting up into the fuel rail. That is why the fuel was heavily aerated after 5 mins, but ran great once all the air was bled out. No need to balance.
