I have the card but can’t find the long form. In the process of switching jobs and the new employer needs it. Is their a was to get a copy of it. Tried googling and couldn’t really find an answer

Why does the employer need it? They have no legal requirement to collect or view the long form, only the medical certificate, and even there only if it has been less than 15 days since you got it or it isn't ahowing when they run you CDL abstract.
Contact the medical examiner that performed the exam, however if you didn't pay for it and your previous employer did then they likely will not release a copy to you.tscottme Thanks this. -
I just had this happen. Just go back to where you did your current physical and ask them to make you a copy. They are required to hold onto them for 3 years. They wanted a 10 dollar fee for a copy but waived the fee as I only have 5 months left on the current one.

All we get is the form anymore.

I'm reading between the lines and wondering if maybe this new employer is wanting you to get another physical. If this is true unless there is a reason you fear doing another physical the best solution is just to let them send you for a physical. If you fear getting another physical then that is a whole different situation.
