I may be going about this the wrong way but I use a printed form that has space at the top for trip number and start date, then the rest is lines for city, state or state line, what I am doing (ie: fuel, lunch, sleep, load, unload, etc) and odometer reading. They are half a sheet of paper size and sit on a clip board. At the end of every load I take the ones used and turn them in stapled to the BOL to the office. That has all state info, route info and fuel info. It can be a hassle to write while driving but it ends up being the exact info that is needed for IFTA and IRP. We havent had a complaint yet.
