We have one Union Contract that calls for OT after 8, 1 1/2 from 8 to 10 hours and double time after 10. And that's Canada wide.

BC Provincial Double time doesn't kick in until you reach 12 hours.

How would it work for OTR drivers, ones that leave Sunday night, and return home Friday night?

How would a company determine the amount of hours worked? Should a driver be paid the minute they leave the yard to the time they return?

I got paid off my log book at one place, it's on duty hours or time that I write down as off duty that's still billable to the company like waiting to unload but I'm in the bunk for 2 hours. You shouldn't get paid for your sleeping time... But I also got a live out allowance.

You are 100% correct. Even a single trip permit out of Province will make you a federal carrier.
