    Had this posted in one of the many threads asking about DAC and have copied it here at CyberGal's request.

    The DAC is a fairly standard format. If I knew how to do it I would post a copy of mine but I will give you the line items that the company has to fill in. The form info is in bold. They are:

    Driver: driver's name
    SSN: driver's ssn
    DOB: driver's dob
    Contributed by: name of the company or companies if more than one under the DOT number
    Phone: company's phone number
    Original data received by DAC on: The date the information was submitted to DAC
    Period of service: Start to finish
    Eligibe for rehire: company will list if they will rehire or not (mine says, "Review required before rehire")
    Reason for leaving: here the compan would list if you were terminated or in my case it states I resigned
    Status: tells whether the driver was a company driver, lease operator, or owner operator
    Driver's Experience: list what all you did while there. Mine states I had mountain driving, OTR, Single driver, and freight handling
    Equipment Operated: Dry Van/Reefer/Flat/Step etc.
    Loads Hauled: tells what you hauled. mine says general commodities
    Work Record: here is where the company can tell what you work ethic/record was like. Mine states satisfactory

    Accident/Incident record


    Equipment was involved in an occurrence or act that produced unintended injury, death, property damage of any type, or reulted in the equipment requiring a tow (other than mechanical breakdown) while assigned to the driver regardless of fault. Adverse information is reported for 7 years.


    Number of DOT recordable accidents less than 7 years old: Here a running total is kept on the driver based on input from the company
    Number of non-DOT recordable Accidents/Incidents less than 7 years old: Here a running total is kept on the driver based on input from the company
    In my case both showed 0.​


    No Additional accident/incident information available. (if there was other reportable incidents they would be listed here.


    Again, it is a fill in the blank format with limited room for data to be submitted. So it makes one wonder what all the fuss is about. Certainly under "Reason for leaving" if the company lists you were terminated or that you abandoned equipment that would weigh negatively when viewed by a prospective employer. There just doesn't seem to be alot ofroom in this format to put a bunch of false info.

    The DAC report also comes with a copy of your MVR(s) and your criminal record.

    DAC falls under the Fair Credit Reporting Act and as such you are entitled to a free copy once a year. You can also request a copy anytime a company denies you employment and lists DAC as one of their reasons for denial. Also, you have the right to have information corrected if it is errorneous and to rebut comments made by a company.

    You can go to the USIS website and download a form to request a copy of your DAC report. Complete the form and mail it in and in about two weeks (they say up to 15 business days) you will receive your DAC report. Review it and if there are errors or comments you wish to rebut you can download a rebuttal form from the USIS website. You submit that to USIS. It is my understanding regarding erroneous entries (wrong dates, reason for leaving, etc) that the company has 30 days to provide proof that their information is correct or USIS has to remove it as unverified. If the company stands by their entry, it will remain there for 7 years (indefinite in some cases like drugs or felonies). However, your rebuttal statement will be listed on the report as well for prospective employers to review.

    There are organizations, just like with credit reports, who advertise they can "fix" you DAC report. There is really not much they can do that the driver cannot do him/herself. They cannot get items removed off the DAC that a company says is true. They may try to fight those comments but the driver can do the same. I guess the only reason to use a service is to save having to do the work yourself. I prefer to keep as much money in my pocket as necessary.

    I hope this is helpful This is what I have learned. I know there are others out there who might have more information to share.

    The biggest thing to remember is that it is important for you to get your free copy once a year, just like you should be doing with your credit report.​

    Just got my cdl and some prehires but was wondering if they run background checks etc before sending prehires and if not how far do they go back when they do run them also take blood pressure meds any problem with this?

    Welcome to the forum!

    The background checks depend on the company. Some companies have everything done before having you come to orientation. Others wait until you are there. Best thing is to ask the recruiter.

    As for blood pressure meds you will most likely be given a one year medial card. It is best to bring info from the doctor who is treating you how the meds are managing your problem. This way, if the BP is up during the physical you will have proof from your DR. that it is normally under control.

    Plus you can get a free one anytime a company turns you down and claims it had something to do with DAC. Due to the free every 12 months, it should be handled like a credit report and one obtained each year since it includes MVR and criminal info.

    Hey guys I'm new and I have a quick question. I have a felony charge back in 2001, however I requested my DAC report from USIS and it was not on there. I applied at a company and they told me that they ran my background and my 5 year employment check. How are things looking for me???

    i think they can use other means to check background not always dac not sure about it though. good luck and hope for the best

    Whether they found it on your DAC or not. Doesn't mean it's not there.

    They may not have gotten that part. Your DAC contains multiple sections. Some companies don't subscribe to all. Some don't even use it.

    They may rely on another service, which obviously hasn't turned it up either.

    thanks guys. I hope to get some OTR exp under my belt without them finding out. I applied to 30 plus companies telling them the truth and got turned down. So they leave me no choice...
