First off the OP should contact the IRS and the IL state tax board to turn this company for for doing the 1099 scam.
Then secondly, he should photocopy that document and send it to the IL state department of labor.
In most cases contracts that have fines set up for employees are not legal even if they are signed, I'm sure the state DOL would find that page interesting.

Tell them you will sign there's if they sign your contract.
My wife gave me that one.
They should have to sign off on paying for rude behavior to driver,
poor equipment,late pay,maintenance,
late night calls,not answering phone,rude dispatchers,poor loads.
You get the picture.I would love to see how they handle that.
You ask them to pony up,and see who flinches first.
I would also need more definition for some of those joke fines.
Company write ups,and sign offs should also have to sign your
version of how events happened in your opinion.
If they wont,then don't sign theres.Tit for Tat -

When I came back, I was here 6 years previously, I already knew the deal. And they aren't hard to deal with. There are some things that are "forgotten about" when you're a good runner. Anytime a company literally wants you to sign your life away, it's bad news. Some of those rules make sense, but to charge hundreds of dollars???? I think not. If you go out of route here, like you're going from Minnesota to Florida and decide to stop home in Ohio, they are going to charge you $1 a mile. If you get caught driving without a hands free device by the LEOs, you're paying your fine AND theirs. I knew this. But I have a Bluetooth so it is all good."Hang - Man" Thanks this. -

I really would love to see the DOT roll into Chicago and start shutting them all down. Most aren't legal at all, not even a little.
