Really what you have is one preventable accident. Again not preferred but hireable nonetheless.

They gave me a chance after no one else would hire me for being fired from my last company for three safety violations. All three were bad. One jack knife,a" runaway truck (yellow valve it's important to pull)and a run under. So I would say they are all about 4th chances. But safety will watch u like a hawk. Good luck!
