One of our trucks has this engine in it. Symptoms past week or so have been poor mpg's, lower than usual power, a "cough" or skip when pulling a hill, and now today truck started blowing black smoke especially when shifting.

However, with the engine brake shut off, the truck seems to run normal, at least it did for the last 3 or so hours today.

I'm assuming this is an engine brake issue? Or other ideas?

Could be worn components in the jake assembly or an actuator or switch problem.

Probably the egr valve is sticking or turbo control box on its way out.
Do you have a boost and pyro gauge?
    Any white smoke?

Boost gauge yes, pyro no. Didn't see any white smoke, just black.

When the engine coughs, boost gauge goes down to about nothing. But soon as I turned the jake brake off, was like I had a new truck for rest of day. Power was great, mpg's increased.

So the black smoke disappeared when you turned off the jakes?
Does the motor have the egr valve?

Yes, smoke went away soon as I switched Jake off. Prior to that, was leaving a huge cloud of smoke.

Yes, one of first EGR engines from early 2003. Just not sure if this is EGR, turbo, or Jake related.

The black smoke and cough is most of the time egr valve sticking open.
Black smoke is too much fuel or not enough air.
How much boost are you making?

Sounds more like an egr valve partially stuck open. A sticking jake solenoid would produce a noticeable miss.

egr over pressure sensor passenger side. two bolts take it out and blow it out with canned air....use compressed air to blow out the two metal lines connected to it.....had a 07 had the same issues sensor is around 100 bucks they need to be blown out ever 50000 miles or so.....or get a few extra/ had so many issues with the egr system i got rid of it

Interesting. I'll thicken the plot: The exhaust gas pressure sensor was replaced 5 days ago. Thanks for the replies.

And yea damil, this truck is history in March, being replaced.
