I have an '08 780 with a d13 that I bought in December of 2012 with just over 500k miles. In the past week (now at 535k miles) or so it's started indicating constantly that regen is required.

I've done parked regens and allowed highway regens but it's like it can't complete a regen. It causes the ECU to throw an error code of MID 128 PSID 47 FMI 1, which leads me to believe the injector or harness connecting to the injector in the dpf assembly is bad. This would maybe make sense, if it's just faulty enough to cause problems but not so faulty that regens are COMPLETELY failing - I think the regens are working somewhat, because the engine hasn't de-rated at all... it just constantly needs regen and alternates between solid "required" and flashing "urgent" on the amber indicator / vehicle message.

Anyway, is the injector in a location that I can get at, and could this be replaced in an afternoon or even a day of working on it? Does anyone have a diagram or pdf of the dpf assembly?

The 7th injector is located on the first pipe out of the turbo assy. It has three lines going to it and it probably is the issue. The thing is to replace it you need to drain the coolant because it is coolant cooled and also there is a 14 mil line for the fuel supply te coolant lines are 7/8 the injector itself has two bolts those are 8 mil you have to look at it from the top to see the mounting bolts and there is one electrical connection that goes behind the valve cover to the driver side of the engine. That said you could replace it yourself as Long as you drain the coolant and are extremely careful with the mounting bolts because they like to break due to how small they are if you can remove a broken bolt have the tools to tap it then you can do if you can't then I would definately suggest taking it to a shop and buying new mounting bolts also make sure they anti seize the bolts when they put them in reason being is that this injector has to be removed and cleaned out with carb cleaner every 100 k or so so u don't have to replace it again let me know if you need any further help

Looks like i'm lucky,; I own exactly same model 2008 truck same motor, have 1,088,000 , miles and never cleaned or replaced the 7th injector: last year i replaced the DPF filter and EGR valve for first time. After 2010 Volvo upgraded the REGEN software,my truck is running Regen only once twice/ week after aprox 2000 miles; maybe less presure on this injector. I clean myself with compressed air EGR cooler, (still original ) and the boost and differential pressure ;this sensors got me a lot of headache getting clogged with carbon active faults.
