Nobody is really gonna like what I have to say here but it’s on the driver. Why? Because I’ll never believe in a million lifetimes the truck was driven that far without some sort of obvious sign. If the driver had been paying attention and checking his equipment. Has the truck not been greased or serviced in 9 months? It didn’t go from bolted solid to out in a few miles and was giving signs. When they were noticed running it till it quits isn’t a money saving solution. Was it a bad repair absolutely so. However running it till it falls out is clearly not giving a crap about the new vibration and just turning the radio up.
I’ve seen it happen several times usually grit and debris is on the mating surface of the rear structure and bell housing it comes out after a few miles and the bolts are loose. Or the tech uses not enough impact or not the right extension going back and they are not tight.
So gapped a 1/2 inch or so at top is definitely on the shop. Letting it fall out is on the daily driver all in I’ll concede to equal liability and say each party fixes their own equipment.1951 ford, Dale thompson, W923 and 22 others Thank this. -

I have never been around a auto trans ...
Just put a clutch in a 2015 Columbia that had a 10 speed
I had to wedge a 16 year old 6' 2" bean pole kid up in the engine compartment to get the upper 2 bolts out of the bell housing .. hands down by far the most inconvenient transmission to remove ..
It definitely reinforces my appreciation for PACCAR products
I'am not making excuses for the shop that did the work on the OP's
Just giving my thoughts about Frieghtliners configurationAlbertaflatbed, cke, bzinger and 2 others Thank this. -
They cross threaded my bolts, held, until it didn’t.
Dunno about his, mine never gave sign until i felt vibration bad, i stopped and climbed under and seen transmission had dropped back some.
But if didn’t fall out.
Screwed up input shaft.
I don’t use that mechanic anymore, I’d used him for a few years before though.
Never had to double check his workAlbertaflatbed, Jubal Early Times, bzinger and 4 others Thank this. -
You realized there was something wrong before it left the sceen..
I for 1 can not comprehend how somebody can not realize there is something wrong untill its way to late .. and claim there wasn't some sort of noise or vibrationAlbertaflatbed, cke, bzinger and 3 others Thank this. -
Dealing with salvage part time I’ve seen many aluminum clutch housings crack in half without warning. Bolts on both trans and engine block still tight. Transmissions are heavy and most just hang without any extra support. Add weight of the driveline and those two housings are all that is keeping it upright and in place.
