So the other day while crossing the scales I got called in for an inspection ... it was only level 3 ... the officer gave me a sheet with the inspection details but didnt have me sign anything ... he has the spot circled at the bottom with my name and at the top it says return certificate to agency listed below ... do I sign this and return it? there is no actual address so where do I return it or do I take it to them? this was the SB I75 Findlay Ohio scale

this was my first inspection ...TIA

Should be an address or phone number on it somewhere. If there were no citations given I don't think you have to bother but someone here will know.

If there were any violations you must sign, certifying the violations have been corrected and return ( within 15 days) by fax email or snail mail, whatever choices are given. If no violations were found, no signature is required and it doesn’t need to be returned. Either way keep a copy in the office filed under Roadside DOT Inspections.

If there were no violations you don't need to do anything other than keep for your records. If there were, there should be an address to send to within 15 days. As an owner operator you can just sign and send in yourself. If company driver you sign, send to your carrier and they have to send it in.

    Rideandrepair Thanks this.

That was my thought and experience too but I don't run in every state and I figured it might be different some places. Last level 3 I had was Ohio and didn't have to send in. Usually the officer will tell you.
