DOT Inspection

    So the other day while crossing the scales I got called in for an inspection ... it was only level 3 ... the officer gave me a sheet with the inspection details but didnt have me sign anything ... he has the spot circled at the bottom with my name and at the top it says return certificate to agency listed below ... do I sign this and return it? there is no actual address so where do I return it or do I take it to them? this was the SB I75 Findlay Ohio scale

    this was my first inspection ...TIA

    I had a level 2 in NY and the paperwork had a QR code to scan with the phone and submit online. I also had a small repair to make though.
