Just announced, left engine detached from the plane on takeoff!
Crude Truckin', Stringb8n and 201 Thank this. -
I have a feeling poor maintenance was involved. AAL flight 191 had an engine detach on takeoff at O'Hare in '79. It was caused by improper maintenance procedures by American Airlines

In Flight 191’s case it was maintenance shortcuts that caused that accident-the plane had recently undergone an engine change at AA’s Tulsa maintenance facility.
In this case there was an uncontained engine failure event, which means shrapnel from the engine may have ruptured fuel lines and anything else. Videos from certain vantage points showed flames shooting from engine 1(the left engine)over the left wing as the MD-11 rotated during takeoff!
And this particular aircraft had from my understanding had recently undergone maintenance.drvrtech77, 201, Carpenter Scotty and 1 other person Thank this. -

Was that the one where they mounted the engine to the pylon with a forklift and damaged the pins and pylon?
Saw something like that on Mayday, can’t remember if it was a md11 -
Yup.
In the middle of the procedure the forklift had been powered down due to a shift change.
