So in the end of inspection it says u have 15 days to send the paper to motor carrier. With there blank lines. Signature. / title /. Date. What is the title. What to put there. Can t send by email or fax?

If your o/o just make something up ( title ) but not to the point you get a visit from the fuzz. -

15 days to send to motor carrier? Aren't you the motor carrier?
Are you an owner?
Director perhaps? President? -
Are you using your own authority or leased to a carrier?
If the latter, turn the whole thing in to the carrier's safety department and they'll fill it out and send it in.
If the former, sign it yourself. For title put something like 'safety director'. -
the signature has to be an officer or employee of the carrier and the title indicates what does the person is in the company that signed it and of course the date it was signed.

You need to read that again. The carrier has 15 days to return the completed form to the state. The carrier is certifying that the faults found have been corrected. If you're independent, then the carrier is you. If you're leased to a carrier, I'd suppose whoever the safety manager is gets it from you, then fills it out and submits it. Process to submit varies by state. Nobody here can tell you how to send it in. Read the instructions printed on the inspection then do it. It's not hard.
