I hate to say this but maybe someone should Google how to keep your neck size under 17 inches. I don't know but seems that is their 1st pre requisite at my recent DOT physical, I don't come close to it and wasn't an issue for me. I would assume it's a person's weight control? I eat a lotta my own cooking from scratch and a mix of produce, well I rarely eat in the truck stops but that's me. Maybe some people just have larger neck size, I will admit I think it's a crazy thing their doing also . By the way, when they ask you about how many hours you get for sleep 7 hrs minimum is what their looking for.

My neck was 16.5 when I weighed 220 which is the lightest I've been since 13 I'm just a big guy. My 15 year old son runs cross country and track and plays football his neck is 16.

if you are over 45 they can request it anytime. they also have rules on weight and neck size. OOIDA has a $150 home sleep test in their magazine 'landline'. I think all yo have to do is go into a clinic and get a DOT physical and then you have a card and you are certified but if you have the above disqualifiers you might have to have a test.
