DOT Medical Certificate - Male to Female Transition (Transgender) Hormones

    Hi everyone. My name is Britney and I am a transgender woman about to start male to female hormone therapy.

    I have been driving trucks for about ten years now, and I am currently working locally (which means face to face interaction with my coworkers). I'm not ready to be out to them just yet, and I'm actually searching for trans friendly non-trucking jobs but in the mean time I need to start my transition.

    The effects of the hormones and blockers won't be instantly noticeable so I'd feel safer staying in the closet at my current job while I look for a non-transphobic company to work for (also not in trucking) that means not disclosing that I'm going to be taking hormones and hormone blockers to the DOT when I renew my DOT physical this month because whatever's on that physical the company will know about since they are required to have that on file.

    Having ten years of CDL-A experience, I've worked so hard and I don't want to and cant afford to jeopardize my cdl in case I ever need it (a backup plan in case my non-trucking job search doesn't go so well).

    Does anyone know if estrogen pills and testosterone blockers would show up on a DOT physical?

    My roommate thought they might technically be steroids but that doesn't make any sense to me since blockers basically block natural testosterone and estrogen is the opposite of testosterone (which is a steroid).

    I know there are trans women who are on hormones and trucking and hormones are DOT approved if you have the prescription but if you are CLOSETED, I'm worried that it might show up in a drug screen as a unknown red flag / false positive.

    Advice would be very much appreciated, hostility not so much.

    I would imagine you are doing this under a physician's care. Your Dr would probably be the person to ask. If this change is what you truly want, you shouldn't care what others think (in regards to your company & co-workers).

    Welcome back! (I think?). I believe you've been on here before, hopefully things won't be so...heated this time.

    Disclaimer time! : I am not a medical professional (anymore). My statements are truthful to the best of my knowledge but may contain errors. Don't take it as the Bible and expect it to hold up anywhere outside of the Internet.

    HRT is pretty common regardless of gender. However you should be fine as long as you show that you're under the care and orders of a physician. (You've got a current prescription.)

    Estrogen, even in synthesized form, is a form of steroid.

    I'm not sure what you'd take as a blocker to be honest...

    If I were you, just make sure of the following:

    - always have and carry documentation signed by your doctor.
    - be up front with medical staff, don't try to hide.
    - long as you follow the first two, you should be fine.
    - some forms of HRT are controlled substances. If you're not sure, then ask your pharmacist to see the original bottle. You'll want to look for Roman numerals clearly displayed nearby the name. Class V is the only funky one as it often varies per state. Classes IV-II are controlled everywhere. (Class I is illegal. :p) Controlled substances are MORE LIKELY to show on a drug screening, it doesn't mean they will show, but they are more likely because they are labeled as having addictive qualities.

    In short, you should be fine. Steroids are taken by many people and for just as many reasons. Long as you cover your hind end and have documentation I see no reason that there should be trouble.

    My only curiosity lays with the results. The DOT orders tests at random, during your medical exam, and if in an accident. I would think that the specific analysis results would not be made readily available to your company, but I could be wrong.

    By the way. When seeking advice about medicine, always ask a pharmacist.

    Doctors know enough about what you're taking to prescribe it. Pharmacists know enough to tell the doctor that they are idiots for prescribing it.

    It'd be kind of like asking your electrician why the toilet is clogged!

    Steroids will show up on a urine test if the lab is testing for them. Trucking isn't the NFL, so they're only administering the $40 to $50 test that looks for illegal drugs, narcotics, etc. It's better just to be honest though because you don't know how those hormones will effect your ability to drive or stay awake.

    I'd check with a dot certified(if you can find an endocrinologist who is also certified DOT that's even better) make sure what ever meds you are taking for HRT won't have any side effects that will affect driving or the physical. And don't self-doctor on that stuff. A close friend of mine did that with spiro and estradiol for a long time and it caused her major health problems.
