Ok at risk of asking a "stupid" question if everyone would glance at their latest drug testing form. Where it says collection site, is that the physical address where you gave your sample or is it a different address? Seems like to me a "collection site" would be the physical address where you produced your sample. Not the personal residence of one of the girls who did the testing. Just wondering

The addy is the same as where I did the whiz quiz.

Bravo young man. Another guy that gets it.....healthcare professionals only care for the bottom line. Your health is secondary. And it's only going to get worse. Greed and profit trumps common sense and genuine caring of the individual that requires proper healthcare.

By the way to the lady whose husband got screwed out of a job, I would certainly seek professional help with regards to the law. If you look at the regs, companies can't just willy nilly request that you take a random. They can't just think up an excuse for you to take the test, just because they feel like it.
it has to be before, during or after a safety sensitive procedure, and as you didn't state the circumstances to your situation, may I suggest that you investigate that criteria.
It can be a very important factor in your defense. Don't assume companies know what they're doing with regards to the law.
    many are absolutely clueless.
    Good luck

Off topic a little, but i think i would rather deal with high blood pressure than any medication that causes me to retain that much water. Can't be good for the kidneys or liver.

Until an MRO contacts him it isn't a positive. And it won't be a positive IF there is a valid medical reason not to produce the sample. Firing is a company policy, not FMCSA regulation, that is no safety sensitive work(driving)until the completion of D/A program.

Dr_Fandango44, the entire group of guys that were on the yard waiting on the next haliburton on job to start got tested. There wasn't any situations to our knowledge. The collector is the secretary, she didn't follow any of the protocols such as securing the bathrooms before and after each test. Samples were left Un supervised on the table in the drivers room with other drivers while collector left the room. Test forms are improperly filled out and have been for at least 6 mos we can prove. He was never told he had 5 days to provide a medical evaluation as to why he can't produce on demand which he did try 3 times and it wasn't enough to make the line 2nd sample was left on the table til it was out of temperature, he was told unable to produce was automaautomatic termination because it was a refusal to test. So he cleaned out his truck and came home. He has another job already if this does show on his DAC we are coming out swinging. Hopefully this is a blessing in disguise and we will be better off at the new place and it's a 14/7 schedule so it will be better in that regard just hope the money is right

Something to consider. Go right now or A.S.A.P to someplace and pay for your own test. Getting a test on your own from a DOT certified place will only help you in the long run, either getting a new job or in a court of law.
