When I first got my CLP (Learner's Permit) the truck driving school sent me for my DOT Physical and I was certified for 2 years. Then I got a job with a mega and they put me through another DOT Physical and I was certitfied for 2 years. About a month ago I took a job with a new company and did yet another DOT Physical and was certified for 2 years.
I just now realized that I am supposed to submit my DOT physical to the state of Texas. I remember showing them the card for the physical when I first got my CDL. I haven't submitted either of the last two physicals to the state because it did not occur to me to do so.
I found a form on a website for the state of Texas where I can mail in a copy of my physical. The web page says something about having to submit it within 15 days of it being issued. It is now more than 15 days since I was issued it. Does this mean I will need to do another physical and submit it within 15 days? Should I just go to the DMV and do it in person? I want to get this taken care of on my hometime in about a week and your help is greatly appreciated!

Like Jenn72 said. Email. They will give you instant conformation that they received it.
I just got a letter in the mail 3 days ago about my med card being expired 60 days ago and that my license is just now being downgraded. I “gave” it up back in Dec. so I think you are still safe as far as time. Lol.88228822 Thanks this. -
I *think* you only need to submit it once you renew it after the 2yrs is up. If you hire onto a new company and they have you get a physical and new med card, either someone at the company or the place where you take the physical will send in your info for your new card/physical. Atleast if I remember correctly thats how my boss/safety director explained it.
