If it’s from the top of the motor it’s the camshaft getting a flat lobe
not sure what to suggest if it’s from the flywheel areabzinger and Rideandrepair Thank this. -
Rideandrepair Thanks this.

Are you certain its from the engine? Does it make the noise, clutch engaged/disengage or change at all? Does the noise change or go away under load or with engine RPM? If so what RPM. Does it disappear off idle?
Rideandrepair Thanks this. -

Probably just the clutch/ throw out rattling- Lube everything and make sure its adjusted and run it????
Rideandrepair Thanks this. -
Maybe take the inspection cover off the bottom, see if noise gets louder. I had mine running that way after doing the clutch. Couldn’t believe how much noise it makes. Made a video, thought I had a problem. Lol. Maybe it will help identify the noise if it is the throwout.

On mine it meant the cam was getting a flat lobe at 228000 miles after a complete inframe .
Of course Cummins took no responsibility for it because they didn't do the work.
And I ate the bill to the tune of 6k.Rideandrepair Thanks this. -
bzinger Thanks this.
