I got a letter in the mail today that simply says
My Name
My Address (it's wrong but mailman delivered it)
My city, state and zip
URGENT: ACTION NEEDED
And that's it, the rest of the page is blank, nothing on the back and nothing else in the envelope. I just passed my CDL Class A test with the state today and I came home to find this.
What does it mean? Where can I go to find out more information? Is there a phone number I can call?

Best bet is to contact them via a number on their site.

Does the actual letter have any form of identification, like letter head, with official seals, address or any other form of identification?
Where did the post mark show it was mailed/processed at?
Does it have a regular postal stamp?
It is possible this may not be a real letter from DOT/FMCSA.
Go to the DOT website, find the contact information for your local field office. Call that office. The person you’re looking to contact is the head field agent for that office. They should be able to assist you.Numb, tscottme, Magoo1968 and 1 other person Thank this. -
Just toss it in the trash.
There’s absolutely no reason that the fmcsa would need to contact you.
It’s just a bs phishing scam..couldn’t even get your address right, lol -
Or that call I get a couple times a year from that same guy asking for money for a police foundation or something.
The URGENT: ACTION NEEDED is to urgently ignore it. -

Look at the postage meter stamp. It is showing a zip 40741. This is London Ky. I know of no FMCSA offices in that town. I HIGHLY recommend you NOT throw this away. Make contact with The FMCSA Midwest Service Center in Matteson, IL @ (708) 283-3577. Be ready to send them a copy of that correspondence if they ask. Something seems fishy about this, but let the FMCSA sort it all out. If the FMCSA needs to make contact with you this field office can verify this.
