I did not think it would be that difficult to find a trucking job with all the "shortage" of drivers. That makes me worry some as I have some employment gaps and the last time I drove truck was 2012 and I am soon getting back into OTR again.

3. I've pulled wet and dry freight in dry vans and reefer units.
4. I don't have any endorsements, plain class A license. Should I get more endorsements?
5. I've applied to local, regional, even OTR, with a variety of carriers.

Lynden Transport has a dock-to-driver program. Maybe work on the docks and do some local work and yard work.
You'll need all the endorsements.
