Swift dot physical

    is there a specific form that I need to take to my doctor stating I have no restrictions and is there a limit on how old the letter need to be

    The doctor will evaluate you and he is the one who will make the determination whether or not you have any limitations. Have him write his determination on a prescription pad. That's how I did it and everyone was fine with it. Mine was for back surgery. Although I don't work for Swift.

    ok I had a plate put in to fix the colar bone I know I don't have any limitation since I am still on active duty

    i'm not saying don't get the paper work. by all means get it but the past time i went to orientation, I've got a plate and nine screws in my left femur and the doctor put me back at 100 percent and i wrote it down on the long form physical. i was asked about it but never was asked for a release on it. just lucky or a fluke i guess because i didnt think to bring any papers on it

    You don't have to have a specific form you can call the doctor who did the surgery or the aftercare and explain you need a letter that say you have no physical limitations. They can just write something out. If you can't contact that doctor then go to one now and get a letter but it'll need to specifically address the collarbone issue.

    You can speak to your recruiter and get more information about it. Well, you SHOULD be able to get that information from the recruiter.

    Puh-LEEEEZE!!!

    O/P: my physical consisted of a wizz-quiz...go in well hydrated...an eye test for accuity, color and peripheral, a whisper test, reflexes, quick hands-on check to make sure there's no acute point tenderness, blood pressure, a scope peek in the eyes, throat and ears, height and weight and I was done. Just your basic "you're not going to die tomorrow" test. I'm trying to remember whether they checked my glucose level...they might have...

    But nothing to sweat. I have psoriatic arthritis and they didn't turn a hair.

    so on your physical what do your eyes sight have to be? I start for swift on the 4th and i have gotten my physical already but i now that make you do another one and was just wondering what they wanted for your eye sight?

    They want you to have two. If you have three they get a bit nervous and four gets you shown the door.
