Medical Certificates, what a joke!

    Exception: If there is an error on your card, such as the doctor making a mistake on the date and marking through it to correct it, you must have your long form to back up the card. Via West Virginia DOT.

    I had a clinic in Indiana give me a long form and no card. I ask for the card. She replies that they don't give cards out anymore because too many people were altering them.

    Which will be a problem when you get to the scalehouse here.

    The long form usually does not show a date of expiration, which is required for it to be a valid medical card.

    when i got my current one i asked for the long form and the docs office said i don't need it etc etc.

    The carrier paid doctor is the carriers puppet. The doc will do what the carrier says cause the carrier runs hundreds of drivers through his practice annually.

    The carrier needs the long form for his driver qualification files and he's going to keep it if he pays for it. If you pay then it's yours but the carrier will want a copy for the file. A pissy carrier will want the original , after all, it's all about the carrier.

    The Carrier also knows the long form makes it easier for you to change jobs. He doesn't want that either. that's why he gets indignant when you leave.

    The Indiana card used to be the back page of the long form. It has been awhile since I had one though.

    Most carriers require you to retest with their doctor so them keeping the long form means nothing, Says something about the company that they aren't even smart enough to realize that.

    Just pay for your own, most of the clinic around here charge $75 to $100 for a DOT physical.

    Indiana has required anyone in that state to have a valid medical examiners certificate on file with the state for many years. No valid physical, they suspend your CDL until a new valid one is received:biggrin_2554:.
