When you have to put someone on your insurance for them to ride...is that company policies that ask for that...or is it a d.o.t thing too?

Yes, you have to have the form for insurance and permission. If you are stopped or in an accident and found, there is a ticket issued. You can get the form and have it valid for a year.

There is no DOT requirement for a passinger insurance policy, nor are you required to present one.
There is a requirement for written passinger authorization.
Even if you own your own truck and run on your own authority. Basically, just a letter from either your company or yourself that states that, such and such person(s) are authorized to be in the truck from this date to that date with either a signature from an authorized company person or if you run own your own authority then you.Last edited: May 4, 2014

Then explain the fmcsa rule 392.60
Failure to have the required document is then a ticketing offense under that rule.
