This thread is probably missing a whole bunch of companies. For example, an early 2015 post says:
"They just upped the pay at IWX to .52 per mile as of the first of the year. One drawback is that they have cameras in all the trucks, both facing the road and the driver."
IWX also treats driver benefits as a bargaining tool. Every month you're there, you're required to meet a certain mileage minimum. If you miss it just once, they demote you to square one and take away your benefits. Doesn't matter how long you've been with the company---even a 3-million miler has this hanging over their head the whole time they're there.
Management likes to downplay it: "don't worry, we'll always get you the miles if you're close to missing it." If that's true, then why have the policy? Even if IWX's owner Steve Coulter doesn't actually harbor the belief that all employee benefits should be abolished, imposing that policy on his drivers sure gives the appearance that he does.
Companies currently using driver cameras...

A forum-wide search would probably come up with lots more companies to add. I don't have the time right now but might get around to it eventually.
For now, here's an updated version of the list above (ShipEX and Swift were removed and IWX was added).

Cretes are only if you have been bad. I heard you need to brought in for it. Do wonder how bad you must screw up to get one? But that in my opinion is the best way to have them. Install them in trucks with horrible drivers while leaving the others be
