Sometimes when too much is applied the hook and peg couplers failed and train broke apart. On a mountain that means the engineer must floor the engine wide open to stay ahead of runaway death.
IF any railroad stations in the path expecting a opposite train on schedule (Single track) would get a very special alarm whistle. (Short 0's repeated) and use telegraph to clear the track for miles. If possible.
They did not always make it.
Westinghouse put three systems onto a train. Independent for engine, train braking for the entire train and a form of spring braking when the entire air system fails. (Big hole) it will absolutely stop if there is a break in the train anywhere.

That's the root cause of my anger against this Cuban. He killed people and hurt more. Hes had at least 15 minutes from where he passed that one escape ramp all the way down to Denver west to get stopped.RussianBearTruckeR and mustang190 Thank this. -
The bottom line for me is that it just shows how desperate and critical our need for real driver training truly is. While we protest E logs, treatment from shippers and receivers, HOS, and everything else under the sun, the probem is right before our eyes. It's the one thing I think most responsible drivers can agree on and should demand. Everything else is just BS.
The industry needs better drivers period. The motoring public would appreciate it and support it as well. It's going to cost though. There is a severe driver shortage. The ones at the top just aren't being honest about what they are doing to solve it. We know though.
