if having a tv, computer, dvd player, game system, reading material, cell phone, full trash can, empty pop cans and coffee cups, noticeable body odor, unkept appeareance are going to get me shut down? Does that mean i will have take a 10 hr break,shower, shave, do my laundry, throw away my trash, clean my truck, and surrender all my electronic gizmos at the scale house before they allow me to proceed? Because if not all those things they counted against me will still exist and I will (at least according to these so called experts) still be fatigued. Sounds kinda dumb, huh?

Here is another question. Said physical was preformed on Sep 1 2008 and the meds are dated march 1 2008, if the driver is still taking them then he/she lied on the from and may have invalidated the medical examiners certificate.
But what if said driver was on blood pressure meds on March 1 2008 but for what ever reason no longer needed them on Sept 1 2008 when the physical was taken, dose the driver still list them? On the from it says "For any YES answer, indicate onset date, diagnosis, treating physician's name and address, and any current limitation. List all medications (including
over-the-counter medications) used regularly or recently" What is "recently" yesterday; last week; last month?
Going with your theory, say I get my physical on Feb 1 2009 and at the time I am not taking any drugs and 6 months later I get gout and have to take meds for it and have them in the truck, what am I supposed to do? Go back and get another physical just to list the durgs for my gout? What if after I do that then 3 months later I get the flu...got more durgs for that now so back to the doc for another physical to list them drugs and my gouts meds...before long I'll either be fired for staying at the doctors office all the time or broke.
Like I said....if an officer wants to see my long form I have no problem with them calling the doctor to fax it to them(if they even would). The law does not state that you have to have the long form with you. That why we get a examiners certificate.
I have been asked many times for my medical examiners certificate but never my long form.

Going to my theory on the long form physical. It is dated on say March 1, 2008. You properly recorded the facts and medical history as of the date it was issued. Anything that is done after, by prescription with the doctor is outside the scope of the physical. It remains valid.
My theory as you call it, is nothing more than mentioning the possible problem with not properly recording the drugs you are under during the physical on the day it was issued. Many people start and stop prescriptions.
I should note that during my almost 10 years, I have only run across one DOT that mandated the request for the long form. It may have been isolated. Who knows. -
I got to ask this...how many of you actually carry a copy of your long form physical with you...I dont...mine is in my employee folder at my home office. I am not required to carry a long form so any LEO asking for one is going to be sadly dissappointed when I do not produce one for them...However like posted earlier I have NEVER been asked for my long form..but have been asked for the short form many times durring those quick paperwork checks at the scales.

I carry mine. The last carrier I worked for returned them to us because several drivers had been stopped and the request made for them.

Driver one: Hear about that silly survey?
Driver two: That survey isn't mandatory.
Driver three: The survey will put you out of service.
Driver four: They're talking about making it mandatory.
Driver five: They're putting all drivers out of service.
Driver six: I heard they're making it mandatory.
Driver seven: The cops and DOT are doing the survey.
Driver eight: They're making drivers take the survey and putting them all OOS.
Driver nine: The survey is mandatory.

According to the Lockridge Report on Sirius radio, Indiana has also adopted this fatigue checklist, and federal recognition of it is on the way. One thing to remember: by the very definition of their jobs, police officers are to enforce laws, NOT interpret them! The fatigue checklist is nothing more than an INTERPRETIVE TOOL--and there are judges and lawyers for interpretation of this sort of thing. Furthermore, officers using this checklist are making in a few moments a diagnosis that should ONLY be made by a medical professional. Save yourself the hassle... if they ask you to take a survey, tell 'em NO!

I have indeed been asked for my long form. I carry it in the same clip board i carry my log book in.
