Go to any auto parts store and buy some carpet/upholstery cleaner in a can. Saturate the seats real good, let it absorb into the fabric, then vacuum the seats. Wipe down everything in the truck including the windows with ammonia, twice. Open the windows and leave them open if possible, such as your weekend off at the house and while driving to air it out. Wipe everything down, cabinets, sleeper curtain, floor. Remove the mattress and air it out. Beat it with a broom to get the dust/bugs/chiggers off it and febreze the holy hello out of it. Start the engine and turn on the A/C full blast with the doors wide open for 30 minutes to clear out the system. You want to get rid of the odor, not just mask it. Any previous smoker smoked while the A/C was running and got that nasty smell into the vent system which really fouls up the air. AND, wash the ashtrays.

My trainer smoked (I don't) and I didn't even realize it when I first got in the truck. After I was aboard he smoked outside out of consideration, which was nice.

His system was to spray every absorbent surface (carpet, mattresses) with Febreeze and open a couple cans of this odor absorbent citrus stuff you can get at Walmart. Look for the larger ones, about 4 inches wide. Toss one of those on the floor behind each seat and with the Febreeze you will knock out the smells in very short order.

I know that when working with car dealerships they will sometimes use an 'ionization' unit.

This device is placed in the car, windows closed and allowed to run. After a few (4-8) hours the smells are neutralized. Works very well with cigarette smoke.

I have been in trucks where they think there is no odor. I have yet to be in a truck that does not reek from it. It gets on the steering wheel and the dash and everywhere else.

I was in the last truck for 4 months. Cleaned with bleach, oust and febreze almost 3 times a week. Still stunk when I got out of it.

Regretfully "masking" oder is all you can do if the nicotine residue
    is visible.
We have tried everything from one "cleaner" to the next, even commercial grade steam units. The ion machines* work pretty good if left for 24-48 hrs but the oder returns in short order.
*Ion machine rental is expensive and requires down time of 24-48 hrs.
