Should I give up my lease purchase and buy a relative's truck?

The price on the truck is good, but you'll almost certainly need to spend some money right away on delayed maintenance (DPF cleaning for sure, probably some fluid changes like transmission and rear ends). You'll spend more on maintenance on a 2018 with 300K plus miles than a 2020. So, trading payments for payments.

I'd really, REALLY, want to dig down into this "down time" fear he's got. Where does that come from? Why can't he just set up an account with Ryder and rent when the truck is down? Something is off, there, and I'd want to find out, starting by pulling a vin report at the dealer to see what has been going on with that truck.

The fluids are cheap, couple hundred bucks for a few buckets, the emissions/exhaust system is where it is going to get expensive
