I do know this for my last dot physical for my company at there choice of dr. They checked my air opening at the back of my mouth and measured my neck. Since Im a big guy and met the criteria for sleep apnea testing due to the above two itiems. Well that put me out of sevice for three weeks minimum, two complete nights at a sleep center, receive machine and instrution on use, use machine a minimum of 4 hrs every night for two weeks and then have sleep center read your card and then dr will give you a one month dot physical card. so now working on a diet and exercise cause BMI might be next.

BMI is about the dumbest thing in the world.
A simple math calculator is gonna tell me that a man who is 6'5" and 316 pounds is obese? Are you sure he is obese?

Duane DeBruyne, deputy director of the Office of Communications for the Federal Motor Carrier Safety Administration (FMCSA), "It is anticipated that before the end of the calendar year, this Agency will formally announce a 'Notice of Proposed Rulemaking' [NPRM] that may include elements of the FMCSA Medical Review Board's recommendations covering a broad range of physical qualifications regulations. Once the NPRM is announced via the Federal Register, there is a public comment period as is required of all federal regulatory agencies. Once the public comment period closes, it can be a year or more before a Final Rule is developed, reviewed by the Office of Management and Budget, and published in the Federal Register, at which point it becomes a regulation." But many DOT medical examiners on their own have begun to use the proposed sleep apnea screening guidelines even though FMCSA has not adopted them. Many drivers have reported getting a 30 or 90 day conditional certification pending completion of a sleep study.
So... my best advise is plan on getting a conditional certifcation if your BMI is greater than 33. -
